Product Report Form

As part of the Quality Monitoring System, in the event of any product malfunction, defect or complaint, a Product Report Form should be completed and sent to the LTL Quality Manager, providing a description of the product and the nature of the problem.  The Quality Manager will then process the form and rectify the problem as soon as possible.
